Requests for disclosure, etc. of retained personal information

The Company shall respond without delay and in accordance with established procedures to requests from an individual (or their appointed representative) for the disclosure, etc. of personal information obtained and currently held by the Company ("retained personal information").

Individuals (or their appointed representatives) can make the following requests (1) through (3) to the Company regarding retained personal information.

(1) Disclosure of personal information held by the Company, disclosure of records of provision to third parties, and notification of the purpose of use

(2) Correction, supplementation, or deletion of personal information held by the Company

(3) Suspension of use, erasure, or suspension of provision to third parties of personal information held by the Company

1. Where to send requests for disclosure, etc.

For requests for disclosure, notification of purpose of use, correction, addition, deletion, suspension of use, etc. of retained personal information, please contact "Contact point for inquiries regarding the handling of personal information, related complaints, and consultation" below. After receipt of an inquiry, a "Written Request for Disclosure, Correction, Deletion, etc. of Personal Information" form shall be sent along with a detailed explanation of the procedures for submitting requests.

Upon receipt of the documents, please fill out the provided form and return it along with all other required documents, such as personal identification documents, and the required fee.

*1 "Identification documents of the individual concerned" and "identification documents of the appointed representative" refer to the following.
 1) Driver's license, passport, health insurance card, pension handbook, or any other ID document that shows the same address as that shown on the individual's certificate of residence and that confirms the individual's current address
 2) A copy of the individual's certificate of residence issued within the past six months

*2 "Power of attorney or other document indicating the appointed representative's status as a statutory proxy" refer to the following.
 1) In the case of a statutory proxy, a copy of the statutory proxy's family register and a written court ruling
 2) In the case of an appointed representative, a power of attorney document with the concerned individual's personal seal affixed and the concerned individual's personal seal registration certificate issued within the past three months

3. Fee for requests for disclosure, etc.

The fee for each request for disclosure of personal information and notification of the purpose of use is 1,000 yen (including tax). No charges apply for other types of requests. Please enclose a 1,000 yen postal money order together with the written request documents.

4. Method of response to requests for disclosure, etc.

The Company shall respond to your request by the disclosure method specified on the request form. If it is difficult to respond to your request by the specified disclosure method, the Company shall disclose the information in writing in accordance with Article 33, paragraph (2) of the Act on the Protection of Personal Information. Please allow at least two weeks from the start of the disclosure procedure until the Company replies to you. Please note that there may be further delays in responding to requests due to congestion at the counter or other business circumstances of the Company.

5. Purpose of use of personal information acquired through a request for disclosure, etc.

Personal information acquired in connection with a request for disclosure, etc. shall be handled only to the extent necessary for the request for disclosure, etc. The Written Request for Disclosure, Correction, Deletion, etc. of Personal Information submitted to the Company shall be retained for five years and then disposed of in an appropriate manner. In addition, identification documents shall be retained for three years and then disposed of in an appropriate manner.

6. Reasons for non-disclosure of retained personal information

The Company may decline to disclose all or parts of the information requested, depending on the content of the request. In the case of a non-disclosure decision, the Company shall notify the individual of its decision and the reasons for that decision. Please note that the prescribed fee shall also be charged in the case of non-disclosure.

However, if any of the following (1) through (4) applies, the information does not fall under the category of retained personal information.

(1) Personal data that, if its presence or absence became known, could result in harm to the life, physical well-being, or property of the individual concerned or a third party.
(2) Personal data that, if its presence or absence became known, could encourage or induce illegal or unjust acts.
(3) Personal data that, if its presence or absence became known, could pose a risk to national security, cause damage to Japan's relations of trust with other countries or international organizations, or prove disadvantageous to Japan's negotiations with other countries or international organizations.
(4) Personal data that, if its presence or absence became known, could hinder the prevention, suppression, or investigation of crimes, or interfere in other ways with the maintenance of public safety and order.
